Preparing for your Patient Visit
To save you time during your visit with our practice, print the following forms and complete them at your leisure in the comfort of your home. Bring the completed forms with you to your appointment. If you cannot complete this paperwork in advance of your visit, we ask that you arrive 20 minutes prior to your appointment.

Insurance/Payment
We participate with most national and local insurance plans, including but not limited to Blue Cross/Blue Shield, area HMOs and PPOs, and Medicare. Our Patient Accounts Department will gladly check to see if we participate in your plan and will file your medical claims for you. Co-payments are due at the time of your visit. Although we provide the information necessary for your insurance company to pay for your medical care, ultimately any balances outstanding for more than 90 days are your responsibility.
